SA Holiday Form

Holiday forms are to be completed and signed off by your host employer.  Holiday cannot be taken when you are due to be at college. Requests for holiday must be made at least 5 working days in advance and sent to your Apprentice Mentor – Julia Coulton ( or Nadia Connabeer (

SA Mileage Form/SA Expenses Form

SA will allow apprentices to claim travel costs of up to £25.00 per week. Claims for such expenses should be made on the mileage form for travel by car and the expenses form for any other form of travel. SA Sickness Form

If you have had time off sick, upon your return to work you will need to complete a sickness form and send it to your apprentice mentor – Julia Coulton ( or Nadia Connabeer ( on your first day back. A return to work interview will then be booked in with you.
